There is a $210.00 diagnostics/evaluation fee per leak bottle/capillary; this will only apply if you choose not to have the leak bottle recalibrated once we have received the bottle. Standard recalibration of leak bottles and capillaries will take 12 business days for delivery. Thank You

By agreeing to terms, you are "digitally signing" this form and authorizing Cincinnati Test Systems to proceed with your Return Merchandis Authorization request.

If you are using a purchase order, your order will not ship out to you until we have received an updated PO with a bill to, ship to, ship via, N30 terms, and the correct amount on it, faxed to us. We cannot accept requisitions, verbal PO#s, or print screens.

Return Authorization Policy

Cincinnati Test Systems, Inc. has developed a Return Authorization System. We have found the system has helped us be more accurate and has also decreased our turn around time. We are now making this our return authorization policy. If you would like to return any item to Cincinnati Test Systems you will first need a return authorization number (RMA#). Any items received without an RMA# can be refused in shipping and shipped back to your plant at your cost. We feel this system will help us to better serve you. Please see instructions below for obtaining an RMA#. Thank You

  1. Call the CTS Service Department at 513-202-5108 and request an RMA form or go to www.cincinnati-test.com and download an RMA form.
  2. Fax your RMA form to the Cincinnati Test Systems Service Department at 513-842-8742, or email it to service@cincinnati-test.com.
  3. You will receive a return fax or an email with your shipping instructions and RMA# to ship the items back on.
  4. Place items in proper shipping box and include a copy of your RMA form inside the box.
  5. Write your RMA# on the outside of your shipping box. So that your item is not lost.
  6. Cincinnati Test Systems will fax or email you a quote of your repair as soon as the items are received and checked out.
  7. A PO will need to be faxed into the service department, or emailed to the service department, with a bill to address, ship to address, N30 terms stated on it, a method of shipping the item back to you, and the correct quoted amount printed on the PO.
  8. Leak bottles can only be shipped via Federal Express Overnight or Federal Express Overnight Collect from our facility. UPS will not ship the leak bottles from our facility.
